The Office of Health, Safety, and Environment (HSE) is excited to announce an important update to our incident reporting process.
To enhance efficiency and streamline our reporting and investigation process, the HSE incident Report form has been recreated as a JIRA form and is now accessible through the HSE page on the MacEwan Help Centre.
Effective immediately, all faculty, staff and students are required to use the new online form to report all incidents. The previous form is no longer available, and a link to the new form has now replaced the old form on Forms Cabinet.
To access the new HSE Incident Report Form:
- Visit helpcentre.macewan.ca and log in or create an account.
- Once logged in, click on “Health Safety and Environment”.
- From the Health Safety and Environment Page, Select “HSE Incident Report Form”.
- Fill out the form and click “Send” to submit it.
