A privacy breach occurs when there is loss, unauthorized access to, or disclosure of personal information in physical or electronic form.

In the event of an incident, please complete the updated Privacy and Information Security Breach Incident Form as soon as possible. You can also find the link externally on the Forms page of the Information and Privacy Office website or internally through the Forms Cabinet.
