Health Spending Account (HSA) – submitting claims for January 2015 payment
For Health Spending Account eligible employees:
This is a reminder that any health spending account claims to be paid in January 2015 must be submitted by December 31.
Health/Dental Plan Rate Reimbursement forms that are received and date stamped by December 5 will be processed by Payroll for the January 2015 payment. Health/Dental Plan Rate Reimbursement forms received and stamped after December 5 will be processed for the April 2015 payment. Please ensure when forwarding forms to Payroll for processing that you provide appropriate contact information including your campus address and phone number.
You may also submit for the reimbursement of your health and dental plan premium rates by attaching a copy of the applicable pay advices to the Alberta Blue Cross Health Spending Account claim form. NOTE: If you only claim your premiums on an annual basis, you can submit your most recent pay advice that indicates the year-to-date (YTD) premium amount that you are claiming.
You may block out any information on your pay advice that you do not want viewed by Alberta Blue Cross except for the following:
- Your name
- Amount of health and dental premiums paid (employee portion only)
- Employer name and pay period dates
For employees submitting Health Spending Account (HSA) claims in time for the January 2015 payment run, please send your claims directly through internal MacEwan University mail, addressed to Alberta Blue Cross, as Print and Mail Services uses a government courier (which is not associated with Canada Post). This will allow your claims to arrive at Alberta Blue Cross in a timely manner. Please remember that claims addressed to Alberta Blue Cross and submitted through internal MacEwan University mail do not require stamps.
